Restaurant inspection reports focus on two types of violations: red and blue.

Red violations are the most serious and are most likely to cause food-borne illness. The violations must be corrected immediately or within a deadline set by the health officer.

Blue violations relate to cleanliness and facility conditions, and are corrected on a set compliance schedule.

A reinspection is required if:

  • Red points exceed 45, or
  • Total red and blue points exceed 65

If red points exceed 100 and inspectors find poor managerial control, the establishment may be shut down. Closures can also happen at any time if there is an immediate health hazard, regardless of score. Reopening requires proof that all violations have been corrected.

No Person in Charge (PIC) was present. Employee covering from another store was unfamiliar with specific processes. A trained PIC shall be present at all times. Train employees before starting shifts.

Multiple food worker cards were missing. Current food worker cards shall be available on site. Establish a system to track and store cards within 14 days.

Sandwiches, milk, and pico de gallo were observed at 42–48°F in various cold-holding units. Time/Temperature Control for Safety (TCS) foods shall be held at 41°F or below. Move pico de gallo to a colder unit. A hold order will be placed on the sandwiches, which were at 48°F.

Vomit and diarrhea cleanup plan and kit were not available. A cleanup plan and kit shall be available and ready for use. Create both before the next routine inspection.

Milk, half-and-half, potatoes, meat, and lettuce under prep unit by stove were observed at 43–49°F. TCS foods shall be cold held at 41°F or below. Corrected on site (CDI). Employee lowered the unit temperature. Monitor throughout the day; service or repair if issues persist.

Back-of-house warewashing temperature was 153.6°F. Warewashing equipment shall reach and maintain the required temperature per manufacturer recommendations. Service immediately and ensure proper temperature is reached.

Lemons for lemonade were washed in a private residence. Home storage and preparation of food is not allowed. Food establishment plans to use the school kitchen for produce washing going forward.

Repeat red violations indicate a lack of active managerial control (AMC). Facilities shall have managers and Persons in Charge (PICs), who actively address violations and ensure long-term correction. Complete compliance schedule and retrain employees.

No item stamp or timer was used for boba under Time as a Public Health Control (TPHC). TPHC foods shall be documented when removed from temperature control and discarded within 4 hours. Employee stated the discard time was 3:00 PM but failed to press the timer button. Corrected on site (CDI).

Unmarked cleaning solution was left out in the facility. Chemical solution bottles shall be properly labeled. Label spray bottle immediately. Corrected on site (CDI).

No vomit and diarrhea cleanup kit or plan was available. No written TPHC procedures were available on site. Written procedures shall be available to all employees, along with a vomit and diarrhea cleanup kit. Email a copy of the procedures before reinspection.

Bags of non-dairy creamer were stored on the hallway floor. Food shall be stored at least 6 inches off the ground. Move products or provide shelving.
